• The Do Something Tour Documentary (the "Film") 

• KP Pictures LLC, represented by Kourosh "KP" Parsi, and Facts Still Matter, Inc. and the Do Something Tour, represented by Steve Dodge (collectively, "Producer") 

This Appearance Release and Consent Form (the "Agreement") is entered into by and between the Producer and the undersigned ("Participant"). In consideration of the opportunity to participate in the Film and for other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, Participant agrees to the following terms and conditions:

Verification of Age

I hereby confirm that I am 18 years of age or older.

Grant of Rights

I, the Participant, hereby irrevocably grant to Producer and its respective licensees, successors, and assigns the absolute, worldwide, perpetual, and unrestricted right and permission to photograph, film, record, broadcast, exhibit, digitize, display, telecast, copyright, use, and otherwise exploit my name, likeness, image, recorded voice, performance, picture, caricature, nickname, biographical information, and any materials furnished by me (collectively, the “Appearance”), in whole or in part, in any and all media now known or hereafter devised, throughout the universe, in connection with the development, production, distribution, exhibition, advertising, marketing, promotion, and exploitation of the Film and any and all ancillary or derivative products related thereto.

Assignment and Ownership

I hereby assign to Producer all rights, title, and interest I may have in and to the Film and my contribution to the Film. I agree that Producer shall be the sole and exclusive owner of all rights in the Film and the results and proceeds of my Appearance, with the right to assign or license these rights to anyone at its sole discretion and without restriction. I shall have no right of approval, no claim to any additional compensation, and no claim (including, without limitation, claims based upon invasion of privacy, defamation, or right of publicity) arising out of any use, alteration, distortion, or illusionary effect or use in any composite form of my Appearance.

Consideration

I acknowledge and agree that I have received good and valuable consideration for the rights granted herein, which may include the opportunity for me to appear in the Film, and I acknowledge the sufficiency of this consideration. I understand that I am not entitled to any further compensation.

Release and Waiver of Liability

I hereby irrevocably and unconditionally release, waive, and forever discharge Producer and its past, present, and future parents, subsidiaries, affiliates, divisions, agents, representatives, employees, successors, and assigns (collectively, the "Releasees") from any and all claims, demands, actions, causes of action, suits, costs, expenses, liabilities, and damages whatsoever, in law or equity, whether known or unknown, that I (or my heirs, executors, or assigns) ever had, now have, or may have in the future against the Releasees, arising out of or relating to my appearance in the Film, my participation in the related activities, or the exercise of any rights granted herein. This release includes, but is not limited to, any claims for invasion of privacy, infringement of the right of publicity, defamation (libel and slander), false light, copyright infringement, or violation of any other personal or proprietary right. Representations and Warranties

I represent and warrant that: (a) I am free to lawfully enter into this Agreement and grant the rights set forth herein; (b) my appearance, and any statements made by me, are my own and will not violate or infringe upon the rights of any third party, including any copyright, trademark, privacy, or publicity rights; and (c) I have read this entire Agreement, fully understand its terms, and am signing it voluntarily.

No Right of Approval / Editorial Control

I acknowledge and agree that Producer has the sole and absolute creative and editorial control over the Film and the use of my Appearance. Producer has the right, but not the obligation, to use my Appearance. I understand that my Appearance may be edited, modified, altered, cut, rearranged, or combined with other material in any manner Producer sees fit in its sole discretion, and I waive any right to inspect or approve the finished product.

General Provisions

This Agreement constitutes the entire understanding between the parties and supersedes all prior communications, whether oral or written. If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall continue to be valid and enforceable. This Agreement is binding upon me and my heirs, legal representatives, and assigns.

Signature of Participant

By signing below, I acknowledge that I am 18 years of age or older, have read and understood this entire Agreement, and agree to be bound by its terms.
